"Making Final Moments Matter"
Eligibility
The following criteria determine eligibility for Hospice care:
Patient is in the last phase of a terminal illness.
Patient is under the care of a primary physician.
There is a primary caregiver available to give ongoing care to the patient in the home.
The family lives in the Sierra Hospice service area which includes Chester, the Lake Almanor Basin, Westwood, Clear Creek and the surrounding areas.
